Article III. Waiver of Liability Dance Studio… Same Waiver as Club Memberships

The use of Club USA Dance facilities and any transportation provided by Club USA Dance shall be undertaken by the class

participants, dependent (s), or guest(s) at their sole risk, and Club USA Dance shall not be liable for any injuries or damages to
any class participant, dependent(s) or guest(s), or the property of any class participant, dependent(s) or guest(s) or be subject to
any claim or demand for injury or damages whatsoever, including without limitation, those damages resulting from acts or
negligence on the part of Club USA Dance, its officers, agents or employees. The member for himself/herself and on behalf of
his/her executors, administrators, heirs, assigns and successors, does hereby expressly forever waive, release and discharge Club
USA Dance, its owners, officers, employees, assignees and successors, does agree to indemnify and hold Club USA Dance, it's
owners, officers, employees, assignees and from all such claims, demands, injuries, damages, actions or causes of action. The
class participant does hereby agree to indemnify and hold Club USA Dance, its owners, officers, employees, assignees and
successors, harmless from all such claims, demands, injuries, damages, actions or causes of action. Club USA shall not be
responsible to class participants their dependents, or guests for articles damaged, lost or stolen in or about Club USA, or for loss
or damage to any property including but not limited to automobiles and the content thereof.

NO FOOD POP OR GUM IN THE STUDIO OR OFFICE AREA….NO LOITERING IN STUDIO
Classes are scheduled close together to accommodate all levels of classes therefore please drop students off
quickly trying not to interrupt classes, send notes to the teacher if necessary asking for a phone call when
teacher is finished that day. Dance Studio 303-979-7772 x 106 Trish Reed calls are returned daily ASAP


CLASS OBSERVATION
Students may enter the studio 5 minutes before class time and remain quietly seated or wait in the office area.
Please do not interrupt classes in session. The hallway is also available if you arrive earlier than 5 minutes prior
to class.
Observation may take place in the 1st 10 minutes of class
.
Special events may be scheduled as observation performances.


GENERAL PRACTICE ATTIRE (What to wear!)
Leotard or stretch pants for girls. Stretch shorts and T-shirt for boys - NO JEANS.
Hair pulled back away from face, preferably in a low ponytail. (Think of tumbling rolls when fixing hair)
No jewelry (stud ear rings are safe),
Disney Dance:  tap shoes (elastic sewn together to replace laces) may purchase from Club USA $18.00 or

Payless Shoes or Once Upon a Child, ballet slippers are optional, bare feet best for tumbling.
Dance: Black jazz shoes or dance sneakers. Gillies for Irish / Free movement dance attire. Hip Hop and

Pom Dance, sneakers!   No jeans, pajamas or dancing in socks.
Tumbling: Bare feet or tumbling shoes along with tight fitted clothing is best for safe spotting of tricks.


MISS MANNERS POLICY
Disruptive behavior will not be tolerated in classes i.e. excessive talking, interrupting teacher while teaching.

1X- student warning…2 X- parent confrontations…3 X- students / parent
meeting.


SNOW OR BAD WEATHER
The studio will not necessarily close for snow days, regardless if area schools are closed.
A recording will be listed on the dance studio voice mail if classes are canceled 303-979-7772 ext. 106


HOLIDAYS
General school holiday's we are often opened. We are closed for:
Labor Day, Thanksgiving, and Christmas from December 23rd through January 1st, Memorial Day, July 4th,

the last 1- 2 weeks in August. Any others will be noted in a newsletter.


DUES All dues are non-refundable after the first lesson has begun.
Purchase a class time slot from start through end of session in Bulk (Package) to receive a 10% discount price

shown on schedule (may be pro-rated upon your registration).

A monthly purchase is based on individual class pricing $10 member or $15.00 non-member daily.

Package prices for sessions are due within one month of beginning.  September / Janauary / June
A $10.00 late fee will be assessed after the 15th of each month and student may be asked to call home or

sit out.
Packages are due in full the month they are offered, to assure your child's class placement.
Make up classes must be made up within each session, ask your teachers for best make-up classes.


RECITAL AND ADDITIONAL FEES
All dates, times and information will be sent home monthly in "The USA DANCE Newsletter"!
Small performances may be held at a shopping mall, skate rink, or during class time.
An invitation will go home with students.
December -Separate shows will be arranged, 2 hours in length, approx. date 2nd weekend of December. 

Most likely SW Plaza. Fees: simple costume approx. $30-40.00. Unless ordered for spring show as well.
Spring Show is generally held the 2nd weekend in May.   This is a big production held at Chatfield H.S. or

Columbine H.S.school in the auditorium on stage.
Fees: $5.00 admissions per seat to cover Auditorium, lighting and production fees $40.00 - $55.00 costume
fee. We will ask for a $20.00 Deposit in February. Pictures will be taken within one week of the big show.
Separate fee to photographer. A springtime brochure is available to help reduce your costume fee for the

Spring Production.   Student will receive 50% credit of all sales up to the amount owed for costume.

DVD of the Recital is available and purchased through that company.  Approx. $30.00.
